During the course of your history taking and examination protocols, the parents request information and advice from you regarding a number of health issues including nutrition and immunization procedures, techniques, risks and benefits. They have heard a few media horror stories about possible immunization risks, but the family physician wishes to begin the immunizations shortly. GPs, hospital consultants and private practitioners will also have to renew their licences every five years under plans to be announced by Sir Liam Donaldson. He will call for senior doctors to assess others who are practising in their area to ensure they are not putting patients at risk. Patients will also be asked for their feedback during the assessment process. Sir Liam's report, Medical Revalidation: Principle and Next Steps, will also suggest steps to ensure that doctors keep up to date with medical advances. The annual assessments will look at prescribing habits, adequate assessment of a patient's condition and any personal issues which might affect their work, such as a problem with drugs of alcohol. Wait-time debate continues Pamela Cowan Leader-Post Tuesday, September 18, 2007 Joyce Manz's blood boils when she sees government health-care ads depicting shorter waits for surgery in Saskatchewan. "When the commercials come on TV, anything to do with health care, I could take my chair and throw it at the television ...," Manz said Monday. "I feel very helpless and now I'm very frightened because I'm on morphine." Manz was put on a wait list for back surgery in August 2006. When she called the Saskatchewan Surgical Care Network last week she was told her surgery won't happen until February but she wasn't given a reason for the delay. "Honestly, I don't know how I'll wait until February," she tearfully told reporters on Monday morning. "As far as I'm concerned, anybody who is believing the ads they're seeing on television are being misled. I would like to know, what is being done about the hospital wait times?" To control pain that shoots down her right leg, the 61-year-old Holdfast woman got a prescription for morphine from her family doctor last week. Although the drug controls the pain, Manz is terrified she'll be addicted to the narcotic by the time she has surgery. Despite her deteriorating condition, she hasn't contacted her specialist since she saw him last year. Saskatchewan Party health critic Don McMorris said his party's MLAs have received an increase in calls about surgical wait concerns over the last month. "People seeing these government ads saying that waiting lists are shortened, people aren't waiting nearly as long and that's not what people are experiencing throughout the province," McMorris said. Health Minister Len Taylor acknowledged that surgical waits are long in some areas, but said since 2004, when Saskatchewan and other provincial governments committed money to reduce wait times and improve access, the Saskatchewan Surgical Care Network's figures show the number of people waiting for surgery has declined by more than 1,000. When Regina's day surgery centre opens, the wait for day surgeries will drop further, which will free up operating rooms for orthopedic procedures, he said. He said the Saskatchewan Party misrepresents the status of health care "to erode confidence in the health-care system and therefore the government." When conflicts occur between patients and the system, Taylor said patients should contact their health region's Quality of Care co-ordinator -- positions the government created for that purpose. "The Sask. Party understands and I think choose to ignore the fact that people are on waiting lists as a result of assessments done by surgeons and that surgeons work to establish their waiting lists based on the blocks of time available to them," Taylor said. "If a patient's needs change, they should be constantly in touch with their surgeons for re-assessment and replacement within that surgeon's waiting list block of time." The Regina Qu'Appelle Health Region has a long wait for some orthopedic procedures, which tend to be time intensive, said Trent Truscott, the region's executive director of surgical care services. "It also was one of the areas that was fairly hard hit in the summer because of cancellations due to the humidity concerns," he said. High temperatures and mechanical problems with one of the Pasqua Hospital's chillers forced the region to postpone more than 200 surgeries because of the risk that the humidity could increase post-operative infections. Job action by the Health Sciences Association of Saskatchewan earlier in the summer also resulted in more than 50 surgeries cancelled in the region. Patient underwent Mitral Valve Repair (MVR) in 1999 and has been on Coumadin therapy with no regular follow up of bleeding parameters. Six days prior to admission, patient experienced headache and dizziness, but no consult was made. Instead, patient self-medicated with Bonamine which afforded relief. Three days prior to admission, headache persisted with increased severity, which prompted patient to seek medical assistance at FEU Hospital. Mobic and Iterax were given. Few hours prior to admission, patient was noted to have changes in sensorium and relatives decided to seek consult at Philippine Heart Center. Upon admission, patient was noted to be unresponsive, stuporous, and speechless, with GCS of 7 (E2V1M4). Past Medical History The patient has denies any history of Diabetes Mellitus and Hypertension. As mentioned, she had a history of Rheumatic Heart Disease and had Mitral Valve Repair in 1999. She is a non-smoker and non alcoholic drinker. Hematologic Latest PTPA: INR = 1.02 Act = 98% II.Anatomy and Physiology of the Brain Blood Supply of the Brain The blood supply of the brain derives from the aortic arch via the right innominate, left common carotid and left subclavian arteries. It includes the conducting and penetrating vessels. The venous system draining the brain is divided into vertebral veins that receive blood from the cerebellum. The cerebral veins have no valves. All the veins of the brain terminate into dural sinuses. External Brain Structures The brain is grossly divided into three main areas: the cerebrum, the brain stem and the cerebellum. The largest portion of the brain is the cerebrum. It consists of two hemispheres that are connected together at the corpus callosum. The cerebrum is often divided into five lobes that are responsible for different brain functions. The cerebrum’s surface—the neocortex—is convoluted into hundreds of folds. The neocortex is where all the higher brain functions take place. The cerebellum lies in the posterior fossa, separated from the cerebrum by tentorium cerebelli. It exerts ipsilateral control. Cerebrospinal fluid Cerebrospinal fluid (CSF) is a colorless liquid that bathes the brain and spine. It is formed within the ventricles of the brain, and it circulates throughout the central nervous system. It fills the ventricles and meninges, allowing the brain to “float” within the skull. The Meninges The meninges are layers of tissue that separate the skull and the brain. The Dura mater is the tough and fibrous membrane. The Arachnoid membrane is the delicate membrane and contains subarachnoid fluid. Pia mater is the vascular membrane. The subarachnoid space is fprmed by the arachnoid membrane and the pia mater. Normal Flow of Cerebrospinal Fluid Cerebrospinal fluid is produced in the Choroid plexuses of the ventricle. It flows from the lateral ventricles to the third ventricle passing through the interventricular foramen. Then it goes through the cerebral aqueduct to the fourth ventricle. From there fluid flows to the subarachnoid cisterns through the foramina of Magendie and Luschka to bathe the cerebral hemispheres. It exits through the saggital sinus to be absorbed by the arachnoid villi. III.Pathophysiology of Subarachnoid Hemorrhage (SAH) The term subarachnoid hemorrhage (SAH) refers to extravasation of blood into the subarachnoid space between the pial and arachnoid membranes. SAH comprises half of spontaneous atraumatic intracranial hemorrhages, the other half consist of bleeding that occurs within the brain parenchyma. Intracranial hemorrhage as a whole comprises 20% of all strokes. Nontraumatic SAH usually is the result of a ruptured cerebral aneurysm or AVM. Blood extravasation into the subarachnoid space has a detrimental effect on both local and global brain function and leads to high morbidity and mortality rates. The classic clinical picture of SAH is marked by the onset of very severe headache, tagged as the “worst in life”. Other associated signs and symptoms are loss of consciousness, seizures, diplopia and focal neurologic signs. The early complications of SAH are rebleeding and hydrocephalus. Other complications include vasospasm, neurologic deficits, hypothalamic dysfunction and hyponatremia. Vasospasm from arterial smooth muscle contraction is symptomatic in 36% of patients. Neurologic deficits from cerebral ischemia peak at days 4-12. Hypothalamic dysfunction causes excessive sympathetic stimulation, which may lead to myocardial ischemia or labile detrimental BP. Hyponatremia may result from cerebral salt wasting (SIADH). Nosocomial pneumonia and other complications of critical care may occur. Pathophysiology Diagram Pathological Cycle Resulting from Increased Intracranial Pressure Surgical Treatment Ventriculo-peritoneal Shunting The ventriculo-peritoneal shunt diverts CSF from a lateral ventricle or the spinal subarachnoid space to the peritoneal cavity. A tube is passed from the lateral ventricle through an occipital burr-hole subcutaneously through the posterior aspect of neck and paraspinal region to the peritoneal cavity through a small incision in the right lower quadrant. I need to present an evidence-based nursing project on bleeding protocols in anti-coagulated patients. While I've researched facts, scales used to measure patients risks for bleeds, and interventions, I feel like I'm not getting a good handle on the project and am not doing what's expected (at least the evidence-based part). As part of the project, my group created own own bleeding protocol based on the facts and interventions we collected, which I think is great. However, I think we need something more along the lines of assessments and outcomes. I was thinking of finding literature to measure other bleeding protocols and their outcomes on patients at risk, but I can't seem to find anything. ADHD diagnosis Observations to the assessment of a child with possible ADHD. There is no single diagnostic test for ADHD so different sorts of information needs to be gathered, such as the following: History of symptoms The precise nature of the difficulties, when they were first noticed, in what situations they occur, factors that exacerbate or relieve them. Medical history Risk factors that could predispose the child to ADHD include difficulties and risks in pregnancy and during birth, for example if the mother was in poor health, very young or drank alcohol or smoked or had an extended or complicated labor. Several medical conditions are known to be associated with ADHD. These include fragile-X syndrome, fetal alcohol syndrome, G6PD deficiency, phenylketonuria and generalized resistance to thyroid hormone. Accidents, operations and chronic medical conditions such as epilepsy, asthma and heart, liver and kidney disorders all need to be taken in to account. Also of possible relevance is any medication the child is taking, as well as any adverse reactions they have had to medication in the past. Past psychiatric history: Enquiring about any mental health problems the child has had can help rule out depression or anxiety being behind the symptoms. Educational history: This means the level of their ability and what specific difficulties they have, how they function within their peer group and get on with teachers, and any behavior difficulties such as suspensions or exclusions. Family history: The mental and physical health of the child's parents and other family members can be relevant, particularly regarding the incidence of ADHD or depression. Less then 1% ahs the family history but this is one of the research data base. Social assessment: The family's social circumstances, such as housing, poverty, and social support may all have an impact on the child's behavior.
